Permission to Use Photograph and Video
For valuable consideration received, I hereby grant Metayer Conservatory For the Arts (MCFA) (“Company”) the absolute and irrevocable right and unrestricted permission concerning any photographs or videos that the Company has taken or may take of me, or in which I may be included with others, to use, reuse, publish, and republish the photographs and videos in whole or in part, individually or in conjunction with other photographs and videos, and in any medium, including but not limited to print, electronic, or social media, for any purpose whatsoever, including (but not limited to) illustration, promotion, advertising, and trade.

I understand and agree that these materials will become the property of the Company and will not be returned. I hereby release and discharge the Company from any and all claims and demands arising out of or in connection with the use of the photographs and videos, including without limitation any and all claims for libel or violation of any right of publicity or privacy.

This authorization and release shall also inure to the benefit of the legal representatives, licensees, and assigns of the Company as well as the person(s) for whom the Company took the photographs and videos.

I am a legally competent adult and have the right to contract in my own name. I have read this document and fully understand its contents. This release shall be binding upon me, my heirs, legal representatives, and assigns.

If the person signing is under age 18, there must be consent by a parent or guardian.​
Minor Release (if applicable)
As the parent or legal guardian of the minor participant named above, I certify that I have the legal authority to sign this agreement. I approve this release and agree to its terms on behalf of my child.
