I voluntarily accept these risks on behalf of my child and agree to release and hold harmless MCFA, its teachers, staff, and representatives from any and all claims, demands, or legal actions arising out of or in connection with these activities. I understand that this agreement is binding on me, my child, and any legal representatives.

Photo & Video Release*

I understand that these images and videos may be used for promotional purposes, including on MCFA's website, social media, and other marketing materials.
